1. What is the WellMed Charitable Foundation?

The WellMed Charitable Foundation was established in 2006 by Dr. George Rapier III, a noted geriatrician who is also the Chairman of the Board. The Foundation is a non-profit 501(C)3 organization that supports seniors and their caregivers in our community with a special emphasis on wellness, prevention, and living with chronic illness. It is the philanthropic arm of the WellMed Medical Group, Inc. headquartered in San Antonio, Texas.

2. What are the major focus areas for the Foundation?

The WellMed Charitable Foundation supports seniors and their caregivers in our community with a special emphasis on wellness, prevention, and living with chronic illness.

3. What are the geographic areas where the Foundation provides funding?

The Foundation provides funding in Bexar County and the surrounding South Texas Counties of Comal and Guadalupe where WellMed clinics are located. Additionally, Letters of Inquiry are accepted from communities served by Physician's Health Choice including Travis, Williamson and Hays counties in the Austin area; Nueces, San Patricio and Aransas counties in the Corpus Christi area; Cameron, Hidalgo and Willacy counties in the El Paso and Rio Grande Valley area. Physician's Health Choice also serves Broward County in the Ft. Lauderdale and Treasure Coast area; Las Cruces area in New Mexico; and in Arkansas, the counties of Conway, Faulkner, Grant, Jefferson, Lonoke, Prairie, Pulaski, Saline & White in the Little Rock area.

4. How do we propose a funding opportunity for the Foundation?

A Letter of Inquiry should include information on the following topics in this order:

• Organization's purpose and history
• The goal of the proposed activity
• Proposal activity (what you would like the Foundation to fund)
• Timeline of the proposed activities
• Amount of funding you are requesting from the Foundation
• Name address, phone number and email address of the primary person to contact within the organization

5. What is the review process once a Letter of Inquiry is received?

Upon receipt of your letter, a determination will be made whether or not it meets the Foundation's goals. If it does not, you will be notified in writing. If you proposal is accepted, you will be contacted by the Foundation.

6. How often does the Foundation accept proposals?

Proposals are accepted on an ongoing basis.
